In many circumstances your GP may send us a referral directly. We will then contact you to make an appointment.

Mr Ponosh’s team will go through the simple referral and appointments process with you in detail providing information in writing to you via the post or email.

If you are a new patient to Ponosh Vascular, we will ask you to complete a Patient Information, Medical History and Privacy Form. Mr Ponosh’s office can forward them to you or they can be found below be printed out below to streamline your appointment process. Please return them via the above contacts.
Please bring a up to date list with you to your appointment.
If you have had any prior relevant tests, please let Mr Ponosh’s staff know as these can be easily obtain electronically for you and our convenience
Without a valid referral, you will not be able to claim a Medicare rebate for your consultation with Mr Ponosh. Please ensure your referring doctor has forwarded your referral directly to our rooms, otherwise please ensure that you post, fax or email your referral to us on receipt of this letter.
Prior to your appointment Mr Ponosh would like you to have an ultrasound of your abdominal and leg veins at Perth Radiological Clinic. Keisha from PRC will contact you direct to make this appointment at a location and time convenient to yourself. She will also inform you of any preparation required.
